The first meeting of the National Coordinating Council for Medication Error Reporting and Prevention (NCC MERP) was held at the United States Pharmacopeia (USP) in Rockville, Maryland on July 19, 1995. The Chairperson, William M. Ellis, called the meeting to order at 8:30 a.m. with a brief welcome and opening remarks. Jerome A.
